The following evaluation of a teacher candidate is required according to Wisconsin State Statute PI 34.002. Please fill out the form fully and completely with written evidence for each domain. 

N/A : No demonstration or evidence of the standard 

Unsatisfactory: Absence of sufficient evidence in lesson 

Developing: Presents some evidence of standard

Proficient: Adequate, consistent evidence of demonstration of the standard
